• Guyana Geology and Mines Commission (GGMC) Endorsement of Application for Work Permit Form [Only Required for Persons Working in the Mining Industry] 

• Guyana Revenue Authority (GRA) Application for Taxpayer Identification Number (TIN)

• The GRA Internal Revenue Form

• National Insurance Scheme (NIS) – Guyana Employed Person’s Application for Registration

• National Insurance Scheme (NIS) Application for Compliance Certificate

• Completed Endorsement of Application for Work Permit forms with two Passport photographs of the Employee attached.

• Two coloured copy of Employer’s TIN Certificate

• One coloured copy of the Applicant/Expat TIN Certificate

• Employer’s compliance certificate from Guyana Revenue Authority.

• Employer’s Certificate of Incorporation (Entire Document).

• Two coloured copies of the Applicant/Expat passport with all stamps/visa present.

• Recent police clearance must be a recent one, within a three months window period.

• Temporary Compliance Certificate from the National Insurance Scheme (NIS).
